FMCSA’s Drug and Alcohol clearinghouse
With the completion of the FMCSA’s Drug and Alcohol clearinghouse; employers are now required to complete an Annual query on all Drivers in conjunction with their Annual Review of Certificate of Violations.
We have designated TenStreet as our C/TPA (Consortium/Third Party Administrator). They have created a direct link for our drivers to complete the necessary Limited Query Consent. The completion of this link will create a profile within our Tenstreet dashboard.
Please click on the link below which will take you directly to the web page; check the box “ * By checking the box, I (a) acknowledge that I have read and understand the above and also have been given the opportunity to copy/print it, and (b) agree to use an electronic signature to demonstrate my consent. An electronic signature is as legally binding as an ink signature” and then click NEXT which will take you to the signature page.
Please fill out all of your information; digitaly sign at the bottom and click SAVE
The link is also being added to the computer in the driver’s room
